The 2026 BFMV Physician Leaders Compensation Report offers a helpful resource for benchmarking compensation for physicians in leadership roles. Built from IRS Form 990 filings — a mandatory disclosure for nonprofit organizations reporting compensation for officers, directors, trustees, and key employees — and supplemented with BFMV's own research into each leader's duties, specialty, and employer characteristics, the report offers a look at physician leader compensation segmented across variables not generally available in comparable data sources, including clinical specialty.
With coverage of nearly 3,500 physician leaders nationwide, across 19 leadership positions, 8 revenue categories, and 13 organization types, it is a useful reference point for any organization evaluating its own compensation structures. The figures reflect total pay for physician leaders across their combined clinical and administrative roles, which makes the data particularly well-suited to stacking analyses that examine aggregate compensation.
Major takeaways from this year’s report include:
Average compensation across all physician leader roles was approximately $408,000, a 2.5% increase over the prior year's report. Taken on its own, that figure is a reasonable macro indicator of the market's trend. But it also obscures more than it reveals. Physician leadership compensation is shaped by several distinct variables — clinical specialty, position scope, organizational size, and organization type.
Chief Medical Officers and Medical Directors need to be further distinguished for benchmarking.
The report includes data on approximately 1,300 Chief Medical Officers and more than 1,200 Medical Directors, making these the two most heavily represented positions in the dataset. The data show significant differences in median compensation among physicians with these titles when segmented by specialty, organizational size, and organization type. Having a large, specialty- and size-adjusted sample for these two positions, in particular, helps benchmark physicians in these roles.
Clinical specialty continues to shape leadership pay.
One of the report's more instructive findings is that clinical specialty remains a significant driver of compensation even once a physician has moved into a leadership role. Leaders in surgical specialties consistently command higher compensation than their peers in primary care and other medical specialties. This has practical implications for benchmarking some positions. For example, the title "Medical Director" does not represent a single market rate. It describes a range that depends heavily on the person's clinical background.
Executive-level roles occupy a distinct compensation tier.
The report also documents a substantial gap between senior physician executive roles and other leadership positions. Chief Physician Executives have a median compensation above $800,000, with the 90th percentile exceeding $2 million, reflecting how organizations compensate physicians who operate at true executive scope — with P&L accountability, board-level responsibility, and system-wide strategy.
Organizational scale and type are strong predictors of pay.
Across nearly every position tracked in the report, organizational revenue correlates closely with physician leader compensation: larger organizations pay more. Physician leader compensation also varies meaningfully by the type of organization involved. Hospitals and health systems are the highest payers, followed by health plans and then physician organizations.
